Registration Forms

Students must be registered with the GIMC before they can receive services.

GIMC Registration for Students who are Legally Blind

You can now register students who are legally blind on-line. Log-in on the Book Order page and select the New Student button in the Registration of Students who are Legally Blind group.

GIMC Registration for All Students (Visual/Physical/Organic) Except those who are Legally Blind

You can now register students who have a print disability on-line. Log-in on the Book Order page and select the New Student button in the Registration of all Students Except those who are Legally Blind group.

Patron registration and contact information form. Patrons must be a teacher of the Visually Impaired, Vision Coordinator, Assistive Technology Specialist or the director of Special Education. Contact the GIMC for information regarding patron registration for special situations.

Order Forms

Book Request Form. The GIMC no longer accepts orders for textbooks that are submitted on paper forms. All book orders must be submitted on-line.

 

Material Request Form. The GIMC no longer accepts orders for APH materials that are submitted on paper forms. All material orders must be submitted on-line.
